About NEXO

What is NEXO?

NEXO is a daily medical diagnosis game for medical students, residents, and physicians. Every day a new clinical case is published with progressive clues. You analyze the clues, submit your diagnosis, and receive feedback based on the ICD-10 classification. The goal is to solve the case using the fewest clues possible.

Who is NEXO built for?

Medical students preparing for clinical rotations, residents practicing differential diagnosis, and physicians who want to keep their clinical reasoning sharp. Also useful for anyone studying ICD-10 coding.

Does NEXO replace clinical practice?

No. NEXO is an educational entertainment app. It does not provide real diagnoses, does not replace supervised clinical practice, and must not be used for clinical decision-making. The cases are fictional or adapted for educational purposes.

What languages does NEXO support?

Portuguese, English, Spanish, French, German, and Russian. ICD-10 code search works offline in all 6 languages.

Where can I play NEXO?

On the iOS app (available on the App Store) or in the browser at nexo.wiki.br/jogar. Your progress is synced across platforms.

How the game works

How does the daily case work?

Every day at midnight (Brasilia time) a new case is released. You receive the first clue and can request up to 5 more progressive clues. You only have one diagnosis attempt per day.

What is ICD-10 proximity feedback?

Unlike apps that only show right or wrong, NEXO uses the ICD-10 hierarchy to show how close your diagnosis is. There are 5 levels: exact match, same subcategory, same group, same chapter, or distant. This helps you learn from near-misses.

What are sinapses?

Sinapses are NEXO's internal currency. You earn sinapses by solving cases, maintaining streaks, and referring friends. They can be used to unlock continents (themed case sets) and access detailed clinical explanations.

What is the streak?

The streak counts how many consecutive days you've played the daily case. Playing every day keeps the streak active. If you miss a day, the streak resets. Pro subscribers get 3 streak freezes per month, and Max subscribers get unlimited freezes.

How many clinical cases does NEXO have?

NEXO has 624 clinical cases spanning 20 medical specialties. There are 500 base cases (covering internal medicine, cardiology, neurology, dermatology, and 16 other specialties, with 104 organized by regional epidemiology in Continents mode) plus 124 cases in Multimedia mode with real clinical imagery and auscultation. New cases are added regularly.

What is Continents mode?

104 cases organized by regional epidemiology. Africa, Americas, Asia, Europe, and Oceania, each with its own mix of common and rare diseases. Useful for training reasoning outside your local pattern. Unlockable with sinapses on any plan (Free, Pro, or Max).

And Multimedia mode?

124 cases with real clinical imagery and auscultation. X-ray, CT, MRI, ultrasound, ECG, upper endoscopy, colonoscopy, dermatoscopy, fundoscopy, physical examination, histopathology, blood smear, karyotype, PET/CT, otoscopy, mammography, colposcopy, and cardiac and respiratory audio. How is NEXO different from AMBOSS or Prognosis?

NEXO is a daily gamified challenge: one case per day, up to six progressive clues, and ICD-10 proximity feedback. AMBOSS is a comprehensive medical knowledge library, and Prognosis uses branching scenarios. NEXO teaches differential diagnosis through the ICD-10 hierarchy itself.

Account and access

Can I play without creating an account?

An account is required to save your progress, join the leaderboard, and maintain your streak. You can sign up with Google or Apple ID.

Is NEXO free?

Yes. The daily case is free forever. Pro and Max subscriptions offer additional features such as case archive access, more sinapses, and streak freezes.

What are the subscription plans?

NEXO has two plans: Pro (30 training cases/month, +200 sinapses, 3 streak freezes, groups up to 15 people) and Max (unlimited cases, +500 sinapses, unlimited streak freezes, groups up to 30, up to 3 team members). Both have monthly and annual options.

How do I cancel my subscription?

On iPhone (App Store subscription): go to Settings > your name > Subscriptions > NEXO > Cancel. On the web (subscription via nexo.wiki.br): go to nexo.wiki.br/paywall and use the subscription management portal. In both cases, plan access remains active until the end of the period already paid.

How does the referral program work?

You receive 50 sinapses and 2 days of Pro access for every friend who creates an account using your referral code. The code is available on your profile inside the app.

Issues and support

The app is showing an error or crashing. What do I do?

Try closing and reopening the app. If the problem persists, force-close and open again. If the error continues, contact us at the email below with: the issue, iOS version, and iPhone model.

My streak was lost by mistake. What do I do?

We can't restore streaks manually. The streak is automatically broken when a day is missed. If you believe there was a technical error, send an email with details and we'll check the history.

How do I report an error in a clinical case?

Send an email to contact@nexo.wiki.br with the subject "Case error" and describe the inconsistency. Our medical team reviews all feedback.

How do I enable or disable notifications?

Inside the app, go to Profile > Settings and toggle notifications. You can also manage them in iPhone settings, under Notifications > NEXO.

Privacy and data

Does NEXO collect health data?

No. The diagnoses you attempt in the game are responses to a recreational activity and do not represent personal clinical data. See our Privacy Policy for full details.

How do I delete my account and data?

Open the app, go to Profile > My Data > Delete account. Your data will be permanently removed within 90 days. You can also request deletion by email by writing to contact@nexo.wiki.br with the subject "Delete my account".
