ENARM: how clinical reasoning is tested

Table of contents
Six hours at the venue. On the table, an optical answer sheet for bubbles and the exam's delivery system. Each clinical case brings two or three questions; in each one, one correct option and three distractors. The question does not ask for the isolated name of the disease: it asks for the best answer in the context of general medicine. This is the national gateway to residency in Mexico: the Examen Nacional para Aspirantes a Residencias Médicas, the ENARM, organized within the framework of the Comisión Interinstitucional para la Formación de Recursos Humanos para la Salud (CIFRHS).
NEXO is a daily medical diagnosis game: one clinical case a day, six progressive clues, and ICD-10 proximity feedback across five levels.
This text explains what the ENARM tests, how the 2026 Convocatoria describes the structure, how the score and specialty selection work, what the medical assessment literature says about vignettes with several questions and how to train. At the end there is a table, frequently asked questions and a checklist. Anyone who has read the series on Examen MIR, USMLE Step 2 CK or NEET-PG and NExT will recognize the act of deciding under a case; the Mexican rule has its own duration, score out of one hundred and selection process.
Key points
- The ENARM is the first stage for entering the Sistema Nacional de Residencias Médicas (SNRM); it measures knowledge in the context of the practice of general medicine (Convocatoria 50° ENARM 2026).
- The official structure tests competencies in clinical cases from Salud Pública, Urgencias and Medicina Familiar, and from the four core specialties: Medicina Interna, Pediatría, Ginecología-Obstetricia and Cirugía General; each case has two to three questions, with one correct answer and three distractors (Convocatoria § 9).
- The 50° ENARM 2026 was administered on September 28, 29 and 30, 2026, in six-hour shifts (for example 08:00 to 14:00 or 16:00 to 22:00 at the venues in Puebla, Guanajuato, Michoacán and Yucatán) (Instructivo de ingreso 2026).
- The total score is out of 100; additional points (social service in a high-marginación zone, certain IMSS/ISSSTE cases) only for the categoría mexicana; to take part in the specialty selection, the official page states a total score greater than or equal to 43.0000 (CIFRHS ENARM 2026; Convocatoria § 11).
What the ENARM is
The ENARM is an objective, consensus-based instrument that measures knowledge in the context of the practice of general medicine. In the Convocatoria, it is the first stage of the process for entering the SNRM; other stages include internal requirements and assessments by the health and educational institutions that take on those selected (Convocatoria § 2).
The DGCES of the Secretaría de Salud, within the framework of the CIFRHS and the Comité de Posgrado y Educación Continua (CPEC), issues the annual call. The 50° ENARM 2026 called Mexican and foreign general practitioners interested in taking, for the first time, a university specialization course in the Sistema Nacional de Salud (Convocatoria; official page).
Where it fits in Miller's pyramid
George Miller described four steps: knows, knows how, shows how and does (Miller, 1990). The ENARM sits at the second: application of knowledge to written clinical cases (and to the venue's delivery system). It is not an OSCE or an exam with a standardized patient like the second stage of Revalida, the AMC Clinical Exam or the French ECOS. It is a competitive ranking of decisions under a vignette, in the same family as the MIR and the NEET-PG.
Categories and role in the pathway
There are categorías mexicana, extranjera, SEMAR and Defensa, with their own registration and selection rules. The ENARM constancia de selección is a requirement for entering SNRM receiving units, valid only for the specialty and venue recorded, with validity tied to the cycle of the guía de seleção (Convocatoria § 12). The CPEC states that it does not teach, organize or recognize commercial preparation courses; official information is on the CIFRHS page (Convocatoria § 13.11-13.12).
Demand and supply (context, not a study target)
The 2026 Call itself includes a historical table of applicants and places in the Mexican category in recent editions, to explain restrictions for those who have already completed a specialty. The number changes each cycle; the candidate checks the year's call, not memorizes the table as a personal goal. What matters for clinical reasoning is something else: with high demand, the instrument must discriminate well between those who decide under a case and those who merely memorize more pages.
How the exam works
Cognitive content
Section 9 of the 2026 Call sets the map: cognitive competencies contextualized in clinical cases focused on Public Health, Emergency Medicine and Family Medicine, and on the four core specialties (Internal Medicine, Pediatrics, Gynecology-Obstetrics, General Surgery). The format is a clinical case with two to three questions per case; each question has a single correct answer option and three distractors (Call § 9).
The current call does not publish, in the text of Section 9, the total number of items. Earlier editions (for example 2017 and 2019) went as far as declaring four hundred and fifty questions and about one minute per item; this should not be copied as a 2026 fact without a new official source. What is documented today is the case format, the domains and the six-hour window of the Instructions.
If a commercial bank or a forum states a fixed total for your year, treat it as a hypothesis until CIFRHS publishes the number. Training against an invented total distorts your pace: what you control is the quality of your reading of the case and the coherence between the questions on the same cuadro within the official six hours.
Duration and logistics at the venue
The Instructions for the registration, entry and administration of the 50th ENARM 2026 define six-hour administration shifts: 08:00 to 14:00 (morning) or 16:00 to 22:00 (afternoon) in Puebla, Guanajuato, Michoacán and Yucatán; in Sonora, 07:00 to 13:00 or 15:00 to 21:00 (Instructions § 3). The candidate may only sit the exam at the assigned venue, date and shift.
In the practice set out in the Instructions, the candidate hands in the Hoja para Ingreso y Registro, receives the optical answer sheet, enters the room with only that sheet, answers in the administration system and, when finished, scans the sheet, prints the Reporte de Puntaje in duplicate and signs both sheets in blue ink. Without a signature matching the one on the identification, the exam is cancelled (Instructions § 7-10; Call § 11.1).
Electronic devices (even switched off), USB drives, watches and similar items are prohibited in the administration area; the parcel service stores belongings. Arriving late is only possible under narrow conditions (up to sixty minutes after the start and without the first candidate of that shift having left), and the exam must still end at the official time (Instructions; Call § 13.15).

Score, tiebreak and selection
Total score out of 100
When finished, the candidate receives the Reporte de Puntaje. The total score combines the exam points and, where applicable, additional points only for the Mexican category: for example one point for community social service in a high or very high marginalization zone duly documented; one point for working a year or more in a facility in a high marginalization community; and additional scores not cumulative with each other in the IMSS and ISSSTE cases described in the call (Call § 11.3).
The page for the 50th ENARM 2026 states that those who take part in the selection process are those with a complete file, who sat the exam and have a total score greater than or equal to 43.0000, according to the guide (CIFRHS selection FAQ). This is not a "school passing grade" in the sense of passing or failing the residency: it is the threshold for taking part in the platform for choosing specialty and venue. The actual selection depends on the number of places per institution and specialty and on the order of the score (Call § 11.3.1).
Tiebreak criteria
In a tie on total score, the 2026 Selection Guide applies, in this order: the highest number of correct answers on high-difficulty items; then medium difficulty; then blocks of Internal Medicine, Pediatrics, Obstetrics and Gynecology, and Surgery; then the highest number of correct items; then the highest weighting of the items (Selection Guide; CIFRHS FAQ). This reinforces the weight of reasoning on difficult items and on the core subjects.
After the exam: choosing a specialty
Specialty selection happens in rounds (the first in October 2026, then December and February 2027, per the guide). Once specialty and site are confirmed, there is no change. The certificate is printed on the platform within the guide's deadline; entry still requires the receiving institution's internal requirements (Guide; Call § 12).

How clinical reasoning shows up
Sequence within the same case
Two or three questions about the same case force coherence: the second question usually depends on reading the first one correctly. An early mistake propagates the error, as in the Key Features of the German M2 or the long vignettes of Step 2 CK. Practicing only isolated cards leaves the candidate fragile when the case asks for diagnosis, next test and management in sequence.
In study practice, note in your notebook when the second question saves or condemns the first hypothesis. If you change diagnosis between question 1 and 2 without new data, the problem is not the question bank: it is the stability of your representation of the case. This is the same kind of failure Eva describes when the clinician anchors too early (Eva, 2005).
Best answer, not all the true ones
One correct and three distractors. Two options may seem defensible; only one is the key. It is the same kind of pressure as the Bestantwort and the best/most appropriate answer. Over six hours, fatigue increases the temptation to mark the option that sounds familiar instead of the one that resolves the crux of the case.
Core subjects plus Public Health, Emergencies and Family Medicine
The official map mixes the four core subjects with Public Health, Emergencies and Family Medicine. This shifts training: memorizing lists of tertiary hospital specialties is not enough. Primary care, population risk and emergency prioritization cases appear in the same instrument as Internal Medicine and Surgery (Call § 9).
Immediate score and ranking
Leaving the site with the score printed changes the psychology of the day: there is no waiting weeks for the raw exam score. The contest, however, continues on the selection platform, where slots and assumptions matter. Compared with PLAB/UKMLA or MCCQE, the ENARM is closer to the MIR and the NEET-PG: a national written exam that orders access to specialist training.
What the studies say
Miller places clinically applied MCQs in knows how (Miller, 1990). Key feature problems and vignettes with a series of decisions sample critical steps better than long open cases (Page and Bordage, 1995; Bordage and Page, 2018). The ENARM design (several questions per case) aligns with this logic, even without using the label key feature.
Eva and Norman note that diagnostic error mixes a knowledge gap with a failure to select the right data under pressure (Eva, 2005; Norman and Eva, 2010). Over six hours, the pressure is twofold: time and volume. Those who only review flashcards of isolated facts speed up on the easy simulator and stall when the stem asks for priority, contraindication or the next step.
Studies and reports on the ENARM in the Mexican literature discuss validity, inequity of access and the weight of the exam in the residency system; the candidate should read the convocatoria for their year, not only secondary articles. For clinical reasoning itself, the useful transfer is to train decision chains in the official domains, not to memorize forum tips.
The Guía de selección, by giving priority to correct answers on high and medium difficulty items in the tiebreak, makes clear what the system values when two puntajes tie: discrimination on the difficult item. In training, this advises not abandoning long or uncomfortable cases; they are the ones that usually carry the high difficulty.
Comparisons with the series:
- Against the MIR: both rank for residency; the MIR declares the number of items and a penalty for wrong answers; the ENARM 2026 declares case + distractors and six hours, with no total number of items in § 9.
- Against the M2: the M2 uses Key-Features and an absolute/relative threshold; the ENARM uses ranking and plaza selection.
- Against the EDN: France separates the written exam and the ECOS; the ENARM is only the national written selection stage.

How to train reasoning for the ENARM
Convocatoria and Instructivo before the question bank
Read the Convocatoria and the Instructivo for the current cycle on the CIFRHS page. Confirm the domains, case format, times, venue rules and puntaje rules. The CPEC does not endorse commercial courses; use official sources for the rules of the game (Convocatoria § 13.11).
Simulate six hours
At least one full six-hour block in the final two months, with breaks only at the pace you will use at the venue. The goal is stamina for reading cases and consistency between the second and third pregunta on the same cuadro.
Train in sequences of two to three decisions
For each training case: (1) main problem, (2) short hypotheses, (3) the finding that decides, (4) best next step, (5) what to rule out. Then answer two or three chained preguntas without mentally resetting the case. This mirrors § 9.
Cover Familiar, Urgencias and Salud Pública
Set aside explicit weeks for these axes, not only for the hospital troncales. The official map names them alongside Medicina Interna and Cirugía.
A useful block: a morning of Medicina Familiar and Salud Pública cases (risk, prevention, referral), an afternoon of Urgencias (airway, priority, stabilization), and only then a troncal block. That way the brain does not treat Familiar as leftovers in the final week.
Language and reading the stem
The Convocatoria 2026 does not repeat, in § 9, the quota of preguntas in English that older editions cited. Even so, many aspirants study in Spanish and encounter dense stems. Train yourself to read the full stem before looking at the options: in cases with two or three preguntas, the rush to mark the first option usually costs you the second.
Errors by type of decision
Error log: wrong priority, ignored contraindication, unnecessary test, failure in Salud Pública, incomplete reading of the stem. The pattern repeats more than the name of the forgotten drug.
Quick checklist
- Read the Convocatoria and Instructivo for the cycle on the CIFRHS.
- Map your study by the seven axes of § 9 (three cross-cutting + four troncales).
- Train cases with two to three chained preguntas.
- Take at least one six-hour mock exam.
- Review errors by type of decision.
- Check venue rules, the hoja óptica and the puntaje signature.
- After the exam: read the Guía de selección and the requirements of the target institution.
After the puntaje
Those who pass the participation threshold enter the platform in blocks. Ties use high and medium difficulty before the troncales. Do not confirm a specialty without reading the venue requirements: being selected does not waive institutional documentation (Guía).
How NEXO helps you train
NEXO does not replace the CIFRHS or registration for the ENARM. It offers clinical cases in which the task is to decide the next step under a vignette, the same move that the two-to-three-pregunta case demands. You can train in Portuguese or in Spanish, compare hypotheses and see where the decision diverged. Those already studying for the MIR, Step 2 CK, M2 or аккредитация can reuse the diagnostic chain.
See the Economy. Whoever coordinates a group or league can find the NEXO Closer. Researchers, reviewers and creators find the NEXO Researcher, the NEXO Curator and the NEXO Creators.
Frequently asked questions
What is the ENARM?
It is the Examen Nacional para Aspirantes a Residencias Médicas, an instrument of CIFRHS/DGCES that measures knowledge in the context of general medicine and is the first step to enter the SNRM (Convocatoria § 2).
How many questions does each case have?
Two to three questions per clinical case, with one correct option and three distractors (Convocatoria § 9).
Does the 2026 convocatoria publish the total number of items?
Not in § 9 of the 2026 Convocatoria. Use only what the official document for your year states; do not copy totals from old editions without checking.
How long does the application last?
Six hours per shift, with times that vary by venue (for example 08:00 to 14:00 or 16:00 to 22:00 at several venues in the Instructivo 2026) (Instructivo § 3).
What does the exam cover?
Salud Pública, Urgencias, Medicina Familiar and the four troncales: Medicina Interna, Pediatría, Ginecología-Obstetricia and Cirugía General (Convocatoria § 9).
How does scoring work?
Total out of 100, with possible additional points only for the Mexican category in the cases set out in the convocatoria. The report is printed at the end and requires a signature (Convocatoria § 11).
What is the threshold for specialty selection?
The official page for the 2026 cycle states a total score greater than or equal to 43.0000 to take part in the selection process, along with a complete file and having sat the exam (CIFRHS 2026).
How does it differ from the MIR or Step 2 CK?
All of them call for decisions on a case. The ENARM ties the ranking to the selection of a place in the Mexican SNRM, with six hours and cases of two to three questions. The MIR and Step 2 CK have different scoring rules and curricular role.
Play the case of the day on NEXO and practice the sequence of decisions before the next six-hour block.
