ENARE: clinical reasoning in the residency exam

Table of contents
One hundred questions. Five hours. Four options and a single correct answer. Internal Medicine, Surgery, Gynecology-Obstetrics, Pediatrics and Family and Community Medicine appear in identical numbers; Mental Health and Public Health run through the exam book in an interdisciplinary way. The final score on this objective exam feeds, in the same cycle, the ranking of the National Residency Exam. That is the current link between Enamed (Inep) and ENARE (Ebserh/HU Brasil): anyone competing for a direct-entry medical residency place does not face a second parallel multiple-choice residency exam book; they use their performance on the unified medical training instrument.
NEXO is a daily medical diagnosis game: one clinical case a day, six progressive clues, and ICD-10 proximity feedback across five levels.
This text explains what ENARE is, how the MEC Ordinance and the public notices link Enamed and the selection of programs, how clinical reasoning appears in the one-hundred-item vignettes, what the medical assessment literature says about this design and how to train. At the end there is a table, frequently asked questions and a checklist. Anyone who has read the series on ENARM, Examen MIR, USMLE Step 2 CK or NEET-PG and NExT will recognize the act of deciding under a case; the ENARE rule is its own: a unified selection process for participating programs, with the Enamed score as the basis for direct entry.
Key points
- ENARE is the unified, nationwide selection process for entry into Medical Residency and Multiprofessional Residency Programs and into a Professional Health Area, organized by Ebserh (now under the HU Brasil framework), with support from FGV (MEC Ordinance No. 329/2025; official HU Brasil page).
- For direct-entry medical residency, Enamed results support selection in ENARE; candidates register for both processes according to the public notices of the cycle (MEC Ordinance No. 330/2025; Inep Ordinance No. 413/2025).
- Enamed administers a theoretical exam of one hundred objective questions, four options, one correct, with an identical number per area (Internal Medicine, Surgery, Gynecology-Obstetrics, Pediatrics, Family and Community Medicine), plus Mental Health and Public Health in an interdisciplinary way; duration of five hours in Notice No. 81/2025 (Ordinance 413/2025; Enamed Notice; Inep note).
- In ENARE 2025/2026 (Notice No. 03/2025, direct entry), anyone who does not reach, on the Enamed objective exam, the minimum of half the correct answers is eliminated from the selection process, even if registered; the Enamed score applies eliminatory and classificatory criteria (ENARE ResMed Notice). Always confirm the notice for your cycle.
What ENARE is
The National Residency Exam (Enare) is a unified selection process intended to fill places in residency programs at participating institutions. It is not a public service entrance exam: approval and enrollment in the program do not create a statutory or employment relationship with the organizer or with the institution (MEC Ordinance No. 329/2025; HU Brasil page).
The objectives stated in the Ordinance include democratizing access, standardizing criteria, guaranteeing equal treatment, optimizing the filling of places and reducing the operating costs of participating institutions. Institutional participation is voluntary; those who join sign an agreement and follow the public notice. The process follows the rules of the National Medical Residency Commission (CNRM) and, where applicable, of the National Multiprofessional Health Residency Commission (CNRMS) (Ordinance 329/2025).
There are two main tracks on offer: Medical Residency and Residency in a Professional Health Area (multiprofessional and uniprofessional). This post focuses on the direct-entry medical track, because that is where clinical reasoning under an Enamed vignette weighs directly on classification. The multiprofessional track has its own notice and rules; do not mix up the schedules.
Where it fits in Miller's pyramid
George Miller described four steps: know, know how, show how and do (Miller, 1990). Enamed, in its objective exam of one hundred items, sits at the second: application of knowledge to written cases. It is not an OSCE or an exam with a standardized patient like the second stage of Revalida, the AMC Clinical Exam or the French ECOS. It is a national decision instrument based on a vignette that, in the current cycle, also ranks access to direct-entry residency via ENARE, in the same family as MIR, ENARM and NEET-PG.
Who organizes what
Inep plans and administers Enamed (blueprint, items from the National Item Bank, schedule, results report). Ebserh/HU Brasil organizes ENARE and sets the rules for selection and entry into participating programs; FGV Conhecimento acts as the examining board for the selection process in recent editions (Portaria 413/2025; HU Brasil page). Inep is not responsible for additional stages, appeals or ENARE results; Ebserh does not replace the Enamed Reference Blueprint. They are complementary roles, not the same exam under two names.
Enamed and ENARE: the official link
MEC Portaria No. 330/2025 established the National Exam for the Assessment of Medical Training (Enamed) as an Enade modality for Medicine courses. Among its objectives: to assess performance against the National Curriculum Guidelines (DCN), to verify knowledge, skills and competencies for practice aligned with the principles and needs of the Unified Health System, and to unify the instrument for assessing medical training (Portaria 330/2025).
Article 6 of the same Portaria authorizes participants to use the results within ENARE (Portaria 329/2025), by registering for the selection process and paying the fee according to the notices. Use of the results is restricted to selection processes for direct-entry specialties, as set out in the Ebserh notice (Portaria 330/2025).
Inep Portaria No. 413/2025 details: Enamed results serve to evaluate courses (via Enade) and to support the selection for direct-entry residency in ENARE; the methodological validity of the results for this purpose is three years; the scale and performance levels are published in an Inep technical document (Portaria 413/2025).
What the ENARE notice requires in practice
In Notice No. 03/2025 (Medical Residency, Direct Entry, 2025/2026 edition), registration for Enamed is a requirement; the scores from the final result of the Enamed objective exam feed the elimination and ranking criteria of ENARE. Anyone who, even if registered, does not reach at least half the correct answers on the Enamed objective exam is eliminated (ENARE notice). That threshold belongs to the notice for that cycle: read the notice for the year you are competing in before turning the number into a mythical forum goal.
For the candidate, the implication for clinical reasoning is direct: each Enamed item counts twice in your head, once as a training assessment and once as residency ranking currency. Training "just to pass Enamed" and "just for ENARE" as if they were separate banks wastes the official design.

How the objective exam works
One hundred items, five areas, five hours
Portaria 413/2025 sets the theoretical exam at one hundred multiple-choice questions, four options and a single correct answer, in identical quantity per area. The named areas are Internal Medicine, Surgery, Gynecology-Obstetrics, Pediatrics and Family and Community Medicine; Mental Health and Collective Health are covered in an interdisciplinary way (Portaria 413/2025; Inep note).
Enamed Notice No. 81/2025 describes the Exam Booklet with one hundred questions and a duration of five hours (example schedule: 1:30 pm to 6:30 pm, Brasília time, in that notice's cycle). Items come from the National Item Bank for Higher Education, based on the Reference Blueprint published by Inep (Enamed notice). There are also mandatory questionnaires (Enade student, contextual, exam perception); they do not replace the objective exam in the ranking, but they are part of the administration.
Content and DCN
The exams consider content, skills and competencies from the areas cited, based on the DCN, on regulations and on legislation governing professional practice in the medical field (Portaria 413/2025). In teaching practice, this pushes training toward cases that mix primary care, public health and hospital clinical decision-making in the same exam booklet, not toward a silo of "tertiary wards only".
Official 2025 exam booklets circulate in exam repositories with patient vignettes, tests and management: the typical format is a clinical stem + a question on best management, diagnosis or next step. Use the official Inep booklet when available; commercial question banks are a guess until they match the Matrix.
What not to make up
Do not confuse the current Enamed with the old model of separate exams for each COREME or with earlier ENARE editions that had their own exam board test. The rule in force for direct access is in Portarias 329 and 330/2025, in 413/2025 and in the cycle's notices. If a prep-course summary still talks about an "ENARE exam with X questions" as if it were the only instrument in 2026, check the date of the text.

Classification, vacancies and affirmative action
At enrollment, the participating institution checks documents and deadlines from the call notice. Sending documents late within the call window loses the place even with a good score. This is not a minor bureaucratic detail: clinical reasoning opens the door; documentation and the calendar close enrollment. Treat the ENARE portal and the chosen institution's COREME website as weekly reading during the call phase.
The official page describes the ENARE classification system as similar to Enem/Sisu: in Medical Residency, the candidate applies to a specialty and then chooses the institution; in the multiprofessional track, the choice logic may differ (assess first, choose later) (HU Brasil page). Vacancies come from participating public and private institutions, with recognized programs and scholarships under federal legislation.
ENARE highlights reserved vacancies for affirmative action (People with Disabilities; Black and Brown People; Indigenous and Quilombola People, as described in the evolution on the official page). The percentages and procedures are in the cycle's notice; do not memorize them from headlines (HU Brasil page).
Enrollment calls and waiting lists are published on the ENARE portal and on the websites of participating institutions. The calendar changes; follow the official electronic address of the notice, not only third-party social media.
For clinical reasoning, classification matters because it places each discriminating item in a relative position. As in the MIR and the ENARM, the study goal is not to "ace one silo" but to maximize stable correct answers under vignettes across the five official blocks.
How clinical reasoning appears
Short vignette, best answer
The four-option format with one correct answer favors items on best management, most likely diagnosis or next step. Two options may seem defensible; only one is the key. Under five hours and one hundred items, fatigue increases the temptation to mark the option "that sounds familiar" instead of the one that solves the crux of the case. It is the same pressure as the best answer in Step 2 CK and the MIR.
Five areas of equal weight
Identical quantity per area changes the study plan. Anyone who only reinforces Internal Medicine and leaves Family and Community Medicine for the last week accepts a structural gap. Mental Health and Collective Health, being interdisciplinary, appear embedded in stems from other areas: postpartum depression, population risk, notification, ethics of care. Train yourself to recognize the cross-cutting axis within the vignette, not just the specialty in the chapter title.
DCN and SUS in the stem
Ordinance 330/2025 ties Enamed to the DCN and to the needs of the SUS. In the practice of the item, this can appear as prioritization in the network, prevention, equity or management in primary care. It is not optional "policy": it is part of the cognitive blueprint. Anyone who only trains tertiary ICU cases becomes fragile when the stem asks for the best next step in the basic health unit or in the emergency of the entry point.
Elimination by half the correct answers
The threshold of half the correct answers in the 2025/2026 notice is eliminatory for ENARE. Pedagogically, this does not authorize studying "only up to half": the ranking above the threshold decides specialty and institution. Treat the threshold as the eligibility floor and the rest of the distribution as the real contest.
Useful mental sequence for each item
For each question: (1) main problem in one sentence, (2) two or three hypotheses, (3) the data that decides, (4) what the question actually asks for (diagnosis, test, conduct, priority), (5) eliminate distractors incompatible with the decisive data. If you jump to the options without closing the problem, the well-written distractor wins.
What the studies say
Miller places clinically applied MCQs in knows how (Miller, 1990). Key-feature problems and vignettes with decisions sample critical nodes better than long open cases without focus (Page and Bordage, 1995; Bordage and Page, 2018). Enamed, with one hundred items and balanced areas, aligns with the logic of broad sampling of decisions, even without using the label "key-feature".
Eva and Norman recall that diagnostic error mixes a knowledge gap with a failure to select the right data under pressure (Eva, 2005; Norman and Eva, 2010). In five hours, the pressure is time and volume. Anyone who only reviews flashcards of isolated facts speeds up on the easy simulator and stalls when the stem asks for priority, contraindication or next step.
Van der Vleuten and Schuwirth describe programmatic assessment and the need for instruments aligned with what is meant to be measured (van der Vleuten and Schuwirth, 2005). Unifying Enade/graduation and residency selection in the same instrument (Enamed) is an educational policy choice: the candidate feels the double weight; the system gains a single signal. Regardless of opinion on the policy, effective training is what improves decision-making under case on the axes of the Matrix.
Comparisons in the series:
- Against the ENARM: both rank for residency; the ENARM declares cases with two to three questions and six hours; the Enamed/ENARE declares one hundred items, five hours and a link with course evaluation.
- Against the MIR: the MIR declares the number of items and ordering rules; the ENARE uses the Enamed score and its own notice rules for Sisu-type classification.
- Against the M2: the M2 uses Key-Features and a threshold; the ENARE is a ranking of adherent programs.
- Against аккредитация: different formats and bodies; the common gesture is decision-making under case in a national exam.

How to train reasoning for ENARE
Ordinances and notices before the question bank
Read Ordinance 329, Ordinance 330, Ordinance 413, the Enamed Notice and the ENARE Notice for your cycle, as well as the HU Brasil page and the Inep Enamed portal. Confirm the calendar, eliminatory threshold, direct-access specialties and enrollment rules. A social media summary does not replace the official PDF.
Simulate five hours with one hundred items
At least one full five-hour block in the final two months, at the pace of the exam site. The goal is stamina for reading stems and stability of decision across the five areas, not just clicking speed.
Balance the five areas
Build a weekly calendar with explicit slots for Family and Community Medicine, Obstetrics and Gynecology, Pediatrics, Surgery and Internal Medicine. Include Public Health and Mental Health as cross-cutting axes (one morning every two weeks with only cases from these axes). Do not leave Family and Community Medicine as the "leftover".
Train the type of question, not just the topic
Sort errors into: wrong diagnosis, unnecessary test, premature management, inverted priority, network/SUS failure, incomplete reading of the stem. The pattern repeats more than the name of the forgotten drug. An error notebook by type speeds things up more than passive rereading of the same PDF.
Stem language and distraction
Read the full stem before the options. On long items, the rush to mark the first "almost right" option costs you the fine distractor. If the question asks for the next step and you answer the diagnosis, you got the command wrong, not just the content.
Official booklets and the Matrix
When Inep publishes the booklet and answer key for your cycle, treat them as gold. Compare the type of stem (management, diagnosis, next test) with your bank. The Common Reference Matrix for the Assessment of Medical Training (Inep ordinance on the matrix, 2025) guides what the exam can sample; do not ignore it in favor of third-party summaries.
Quick checklist
- Confirm the Enamed notice + the ENARE notice for the cycle.
- Lock the Matrix/official areas into the weekly plan.
- Do a five-hour mock exam with one hundred items.
- Review errors by type of decision.
- Set aside blocks for Family and Community Medicine, Public Health and Mental Health.
- In the week of the exam, prioritize sleep and rhythm, not a new chapter.
How NEXO helps you train
In researcher mode, you explore hypotheses more calmly; in closer, you practice closing under time pressure; the curator and the Creators area help you review what you have already seen. Usage limits and the daily economy are at /economy. There is no claim here that NEXO raises your score on Enamed or ENARE: what the product offers is deliberate practice of reasoning under a case.
If you already study with question banks, use NEXO on the days when the typical error is "I left the stem too early" or "I anchored on the first hypothesis". A well-played case forces the same discipline as the right item: read everything, weigh the decisive data, only then choose.

Frequently asked questions
Does ENARE still have its own multiple-choice exam for direct access?
In the design of Ordinances 329 and 330/2025 and of ENARE Notice 03/2025 (direct access), selection uses the final score of the Enamed objective exam. Confirm the notice for your cycle: rules may detail enrollment and documentation steps without creating a second national MCQ booklet.
How many questions does Enamed have?
One hundred multiple-choice objective questions, four alternatives, one correct, with an identical number per area, according to Inep Ordinance 413/2025 and the Enamed Notice.
How long is the objective exam?
Five hours in Enamed Notice No. 81/2025 (example time in the notice). Confirm the notice for the year you register in.
What eliminates you in ENARE 2025/2026 even with registration completed?
Not reaching the minimum of half the correct answers on the Enamed objective exam, according to Notice No. 03/2025. Other requirements (diploma, deadlines, documentation) are also in the notice.
For how many years is Enamed valid for ENARE?
Ordinance 413/2025 provides procedures to ensure results are valid for three years for use in direct-access residency selection processes. Details on how results are used appear in the Inep/Ebserh notices for the cycle.
Is ENARE only for federal hospitals?
No. Participating public and private institutions offer positions, provided the programs and positions are authorized and participation follows the notice (HU Brasil page).
How does NEXO fit into preparation?
As practice in reasoning under clues and hypotheses, complementary to the booklet and the Matrix. No promise of a score; with a habit of decision.
